purchase at participating The Home Depot stores in the U.S. only (including Alaska, Hawaii, and U.S. Virgin Islands) between
6/27/09 and 7/5/09.
2. Complete this form and mail along with a copy of the original receipt‡ with an arrow pointing to the qualifying purchase(s).
Please include an original UPC from the package of each qualifying product purchased. Must be postmarked by 8/5/09 and
must be received no later than 9/5/09 to be eligible.
